Pain Management
The Mogen circumcision technique is one of the quickest methods of neonatal circumcision. The procedure is complete within two minutes and, after we pick them up, most babies have calmed down within 10 seconds. Therefore, pain management techniques such as a lidocaine ring block or penile nerve block are not generally necessary. Lidocaine injections probably do have a place with a Gomco circumcision or a sleeve resection method as these take significantly longer. Most parents in our practice (more than 90%) choose to skip these injections for their sons’ circumcisions.
We do, however, recommend applying an over-the-counter 4% or 5% lidocaine cream such as LMX.
The two methods of lidocaine injection are the Dorsal Penile Nerve Block (DPNB) and the Ring Block (RB). Essentially, the nerve block is two injections given at the base of the penis dorsally. The ring block is a circle of injections, usually for our five, given just beneath the skin (subcutaneously).
Proponents of the nerve block prefer it because they feel the ring block can “puff up” the shaft skin. This can make it more difficult to gauge the correct amount of foreskin to excise. Proponents of the ring block prefer it because it more effectively blocks the ventral nerves, those on the underside of the penis. Some practitioners will give a DPNB but also inject a bleb of lidocaine ventrally to address this.
A new paper compares the penile nerve block to the ring block. https://pubmed.ncbi.nlm.nih.gov/38587622/ . This study finds them to be comparatively equal in effectiveness.
